Results of research

The subject of research work:

1. The study of age features in the metabolic adaptation to the influence of damaging oxidative stress factors and create differential methods of antioxidant therapy

Subject: oxidative stress, metabolism of aldehydes, resistance to free radical damage. Objective: to study the role of aldehyde catabolism enzymes as regards age changes of the organism resistance to oxidative stress. Methods: spectrophotometry, spectrofluorimetry, differencial centrifugation. Theoretical and practical results, novelty: in the development certain conditions are formed in the liver and in the skeletal muscle of rats to change aldehyde dehydrogenase and aldehyde reductase function during immobilization stress. The conditions manifested themselves through specific strain related to aldehyde utilization in aldehyde reductase reaction in the liver of 1,5-month-old rats, and through delay in their utilization in aldehyde dehydrogenase and aldehyde reductase reaction in the muscle. The role of these enzymes activation and biosynthesis stimulation is supposed to be these increasing the resistance to oxidative stress.

2. Study clinic-pathogenetic peculiarities of diabetes mellitus process combining with thyroid pathology in children and adolescents for perfecting effectivity of thear treatment

Subject: 362 children with DM, in connection with thyropathies being present. Objective: optimization of approaches to the treatment of diabetes mellitus with concomitant thyropathies in children and adolescents through exact determining the mechanisms of forming combined variants of the disease course in childhood and adolescence. Methods: conventional clinical, instrumental (USI, ECG, RVG, REG), biochemical, radioimmune (T3, T4, TTH, cortisol, STG, antibodies to β-cells), immunologic (T- and B-lymphocytes, T-lymphocyte subpopulations, phagocytosis, immunoglobulins, CIC and antithyroid antibodies). Teor. and pract. results, novelty: there was revealed a significant incidence of thyroid pathology in children with diabetes mellitus, irrespective of sex. It has been proved that presence of autoimmune thyroiditis in patients with type I diabetes mellitus in childhood is characterized by a less prominent loss of insulin residual secretion by B-cells, by persistence of expressed or subclinical hypothyreosis, as well as by certain rise in cortisol content in girls and in patients with autoimmune thyroiditis formed from the moment of diabetes mellitus manifestation. It was established that immune thyroiditis in children was characterized by inhibition of T-cells and phagocytal links of immunity as well as by an increase in the share of B-lymphocyte with an increased formation activity of antibodies to thyroid gland tissue, and insular apparatus of the pancreas, especially due to antibodies to glutamat decarboxilase.

3. Social and medical health monitoring of school children residing in the town of Jhovty Vody as a component in the set of measures to provide protection of younger generation from radiation impact and to prevent the developing of unhealthy habits in those as well as maintaining their health.

Subject: 1992 schoolchildren from the town of Zhovty Vody. Objective: developing a system of socio-medical monitoring of health status in schoolchildren of Zhovty Vody, the monitoring being a component of the set of means aimed at providing protection of the town children from radiation influence and related harmful factors as well as at prevention of development of bad habits in schoolchildren, and improvement of their health. Methods: screening-estimation of their health status and of in-school environmental factors, and socio-hygiene questioning survey. Theoret. and practical results, novelty: there were defined significant socio-hygienic environmental factors, aiding in expansion of smoking, alcohol and drugs addiction, as well as those that influence health forming and health preserving behavior in senior schoolchildren. On the basis of schoolchildren questioning there were established sex and age peculiarities of subjective estimation of one’s own health and day-schedule for junior and middle-age schoolchildren. There was developed a technology of medico-social monitoring of schoolchildren health status (somatic, physical and socio-psychological components) taking into consideration influence of environmental factors within school. There was set up an informational, analytical and resource-providing center of automated socio-medical monitoring of schoolchildren’s health to ensure functioning of the system of continuous monitoring and control of influence of the environment impact on the health status of schoolchildren. A database as regards monitoring the health status of schoolchildren and in-school factors influencing it was worked out. Sphere of application: hygiene of children and adolescents, health care.

4. To work out an algorithm of interdisciplinary composite appreciation of developing children with psychomotor disorders in the system of early intervention

Subjects: 1000 children aged up to 3,5 years, 100 children with psychomotoric development disorders, 100 children without such disorders and their mothers. Objective: improvement of the rehabilitation system for children of early age with psychomotoric development disorders employing the principles of early intervention. Methods: clinical, experimental psychological (Lusher test and color test of relations). Theoretical and practical results, novelty: there was designed a principally new model of interdisciplinary estimation of development in children with psychomotoric development disorders, based on the principles of early intervention and family-focused approach. An algorithm of interdisciplinary assessment of development disorders in children of an early age was designed and approved. It offers an integrated step-by-step realization of diagnostic procedure.There were proposed and tested certain mechanisms of drawing the parents as experts to the process of estimating the child’s development at all the stages. A principally new integrative parameter – the level of functional organization of the child-parents relationship was proposed. It was established that one of characteristic features of mothers brining up children with of an early age with psychomotoric development disorders is a strong inner strain and stress and ambivalency of emotional perception. It was proved that the proposed model of interdisciplinary assessment made it possible to receive information on biological and socio-psychological components of the child development as well as on the factors that influenced the process of the family functioning. Effectiveness: early revealing children with development disorders, disability prevention and social adaptation of children and the families brining them up. Fields of use: pediatrics, rehabilitation, psychology of development and medical psychology.

5. To study clinico-pathogenetic features of osteoarthrosis in adolescents for raising the effectivity of treatment measures

Subject: 86 adolescents with osteoarthrosis. Objective: improvement of diagnosing osteoarthrosis in adolescents on the basis of studying clinico-immunologic manifestations of the disease and disorders in connective tissue metabolism. Methods: clinico-laboratory (immunologic and biochemical). Theoretical and practical results, novelty: osteoarthrosis in adolescents was characterized clinically und rentgenologically. It was determined that in adolescents take place significant disorders in proteoglycanes and collagen metabolism. Close interrelation was revealed between collagen and proteoglycanes metabolism and activation of acid and alkaline phosphatases. There was proved the influence of cytokines and immunologic shifts on osteoarthrosis development. There was established the role of disorders in interaction between connective tissue metabolism and immune system. Differential and diagnostic algorithm was worked out and most informative diagnostic symptoms of osteoarthrosis were determined in adolescents. Effectiveness: the results obtained will facilitate timely osteoarthrosis diagnosis in adolescents and prescription of appropriate therapy. Field of application: polyclinics for children and adults, cardiorheumatologic clinics and cardiorheumatologic departments of in-patient clinics.
